Instructions for Registry
Change of Address Form

This form is used to inform the Registry of Motor Vehicles (RMV) of a change of address. You are required by law (Chapter 90, section 26A) to notify the RMV in writing within thirty (30) days of changing your address. Failure to do so could result in a $35.00 civil fine. You can fill out this form and mail it in or you can complete the form online and submit it electronically to the Registry (no mailing required). If you would rather fill out the form by hand, please read these instructions before downloading this form:

-->Complete this form and return it to any RMV branch office or mail it in an envelope to the following address:
 
Massachusetts Registry of Motor Vehicles
Attn: Address Change
P.O. Box 199106
Boston, MA 02119-9106
 
-->Write your new address on a sticker and place it on the back of your
license. This sticker may be obtained at any RMV branch office or by calling the RMV Telephone Center.
 
-->If you have more than one vehicle registered in your name, enter all plate numbers.
 
-->On your certificate of registration, write in your new address.
 
-->Please make sure all information is correct and legible.

Please note, if you want your change of address to affect your voter registration, you must submit an application for change of information at a Registry of Motor Vehicles licensing location which is found at any RMV branch office or License Express location. Or you may visit your new city/town hall, or contact the Secretary of Commonwealth site for information.
