for FID/LTC
- Change
of Address Form
This form is used to complete a change
of address notification for a license to carry firearms (LTC)
or a firearms identification card (FID) in Massachusetts. If
you move, you are required by law (Chapter 140, section 129B)
to notify the following:
 Massachusetts Criminal
History Systems Board
- Police Chief of the town
which you are leaving
- Police Chief of the town
to which you are moving
You must make your notification in writing
within thirty (30) days of changing your address and you must
send this form via certified mail. Failure to do so could result in suspension or
revocation of your LTC or FID. Please
read these instructions before downloading this form:
- Complete this form by
PRINTING CLEARLY all information requested. Make three (3) photocopies
of this form.

- Make three (3) legible
photocopies of each side of your current LTC or FID. Each side
of the license should be on a separate page and labeled as pages
#2 and #3 respectively.

- Attach one of each page
#2 and #3 to the original form plus two additional copies of
the form. You should have one extra copy of the form by itself.

- Send the original form
with attached pages #2 and #3 via certified mail to:

- Attn: Change of Address
Notification

- Mail one (1) copy of the
form with attached pages #2 and #3 via certified mail
to the police department that issued your LTC or FID.

- Mail one (1) copy of the
form with attached pages #2 and #3 via certified mail
to the police department in the city/town to which you have moved.

- Retain the last copy of
this form in a safe place for your own records.
