- FID/LTC Application
This form is used to apply for or renew
a Massachusetts license to carry firearms (LTC) or firearms identification
card (FID). Please note that the licensing requirements for firearms
in Massachusetts changed drastically in 1998. To review the changes
that may be applicable to you or your situation, visit our FID & Firearms Permits section. Please
read these instructions before downloading this form:
Fill out this form completely
and accurately. Make sure to check off the type of license for
which you are applying and whether this is a renewal or new issue.
If you are unsure which type of license you need, refer to our

page.

Obtain two (2) 1-inch
by 1-inch color photographs of yourself. These should be head
shots that present a reasonable likeness of you (example--if
you normally wear glasses, you must wear glasses in the photos).
Do not wear any hats. These photos will need to be brought with
you when you bring in your application. If you can not obtain
photos on your own, the Hatfield Police department can supply
photos for a nominal fee.

If you are applying for
your first FID or LTC, you must also obtain a Firearms Safety
Certificate or Hunter Safety Certificate and attach it to the

Bring the completed form,
safety certificate (if applicable), photos and a non-refundable
$100.00 processing fee to your local licensing
authority. DO NOT mail the application to the Firearms Record
Bureau. If you live in Hatfield, your local licensing authority
is the Hatfield Police Chief David Hurley. Checks should be made
payable to the Town Of Hatfield. During office hours (see
our Department
FAQ page for office
hour times), the police chief processes firearms applications.
We recommend you call ahead at (413) 247-0323 to make sure there
will be sufficient time and available personnel to complete the
application process. You will be fingerprinted, so it is recommended
you do not wear expensive clothing in the unlikely event that
the ink transfers to your clothes.

Once your
application is complete, the police department will send it to
the Massachusetts Firearms Record Bureau in Boston. You will
be called by the Hatfield Police Department when your license
is ready to be picked up. Please do not call the station and
ask when it will be ready. We have no control over the application
process and due to unexpected backlogs, license returns can be
delayed by several weeks.
NOTE: There is no grace period anymore
for expired LTCs or FIDs. Expired cardholders caught carrying
a weapon can be subject to a fine up to $5000 and/or arrest.
