Motor Vehicle Accident [PDF] This is the Commonwealth of Massachusetts' standard Motor Vehicle Operator's accident form. This is also commonly referred to as the "white form." If you need instructions for this form, please click here.
Registry Change of Address [PDF] This form is used to inform the Registry of Motor Vehicles of a change of address. If you need instructions for this form, please click here.
