On occasion, while interacting with the
Hatfield Police Department, the Massachusetts State Police, or
the Massachusetts Registry of Motor Vehicles you may be required
to fill out some type of form. The most common types of forms
are used to:
Request information
Report an incident or
accident
Conduct a transaction
Apply for a license or
permit
These forms are usually available from
any local or state police agency and some can also be found at
any Massachusetts
Registry of Motor Vehicles branch location. However, by visiting
our web site you can download many of the forms you may need
onto your own computer. Downloading forms saves you time and
is a convenient way to avoid driving somewhere only to discover
the agency is temporarily out of what you need. Our web site
never runs out of forms!!
Most of the forms available here are in
a format known as Portable Document Format, or PDF. This means
the forms were created with a program called Adobe
Acrobat. In order to view these forms on your computer you
will need to download and install a FREE program called
Adobe
Acrobat Reader (unless you already have it installed). You
only need to do this once and then you will be able to download
any PDF document anywhere on the Internet as often as you'd like.

Adobe requires your name and email address at their site to download
this free program. Please note that this information is neither
collected nor monitored by the Hatfield Police Department. You
may also have to close your Internet web browser to install this
program, so simply return to this site when you are done.
NOTE:
All forms available on this page must be printed out on plain
white paper (unless specified otherwise) using the printer attached
to your own computer. You can then type or print on the form
to fill it out and either deliver or mail it to the appropriate
agency. Most of these
forms cannot be filled out online and no information can be sent
electronically to agencies using these forms.Currently,
the only form available to be filled out online is the Massachusetts
Regsitry of Motor Vehicles Change of Address form.
